University Family Physicians at Forest Park participates with many different insurance companies. Although we submit claims to these contracted carriers, you are responsible for any copayment, coinsurance, or deductable amount at the time you receive services in accordance with the terms of your health plan contract.

*IMPORTANT* : Please bring your insurance card with you to every appointment.

• Patients are expected to pay their copayment at the time of service. If you do not pay your required copayment at the time of service, you may be subject to an additional charge of $20
• Self-pay patients are responsible for full payment of charges at the time of service. All charges must be paid unless you have a signed payment plan
